About the role
At St. Croix Hospice, the Social Worker plays a vital role on the patient care team, offering compassionate emotional and practical support to patients and their families in accordance with each individual’s Plan of Care. This position involves meeting with patients and their loved ones, working collaboratively with the clinical team, and helping families access the appropriate community resources and services they need.
Responsibilities
- Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
- Provide counseling, crisis intervention, and bereavement support, including follow-up for at-risk individuals and coordination of bereavement activities.
- Collaborate with the interdisciplinary team and maintain accurate documentation.
- Support cultural and end-of-life needs, liaise with community agencies, and ensure HIPAA compliance.
Requirements
- Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
- Current Social Work license in the state(s) of practice.
- One of the following licensure levels:
- Certified Social Worker (CSW)
- Certified Advanced Practice Social Worker (CAPSW)
- Certified Independent Social Worker (CISW)
- Licensed Clinical Social Worker (LCSW)
- Social Work Training Certificate (SWTC)
- Ability to pass background check, consumer report, and drug test.
- Reliable transportation, valid driver’s license, and proof of auto insurance.

Schedule
Monday–Friday, 8:00 AM to 5:00 PM, full-time position.
