Social Worker / Therapist - In-home Services - South Bend

About the role

Oaklawn provides mental health and addiction treatment across Goshen, Elkhart, South Bend, and Mishawaka in Indiana. We offer various therapeutic roles including therapist, counselor, clinician, nurse, psychiatric technician, behavioral health technician, and substance abuse counselor.

Responsibilities

- Provide individual, group, and family therapy sessions - Conduct assessments and develop treatment plans - Collaborate with interdisciplinary teams to support patient care - Administer medication management as necessary - Maintain accurate and timely documentation of patient care

Requirements

- Bachelor’s degree in psychology, counseling, social work, or a related field - Valid state license/certification in the applicable field (e.g., LPC, LCSW, RN) - Experience in mental health and/or addiction treatment preferred - Excellent communication and interpersonal skills

Qualifications

- Strong commitment to providing compassionate and effective care - Ability to work independently and as part of a team - Flexibility to work evenings, weekends, and holidays as needed
