Social Worker (SW)

Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Ensure patients reach the end of life with their affairs in order, including financial, funeral, and other arrangements.
• Navigate the grieving process for patients' loved ones.
• Grant end-of-life wishes and listen to patients' stories at their bedside.
• Counsel and offer crisis intervention services.
• Provide bereavement support and grief counseling.
• Educate patients and families about the emotional and psychological response to illness progression.

Requirements

• Master's of Social Work from a Council on Social Work Education-accredited school, or equivalent experience where allowable by State Regulations.
• At least one year of social work experience in a healthcare setting.
• Experience working with terminally ill patients and their families.
• Excellent listening and communication skills.
• Ability to be an effective patient and family advocate.
• Empathy and compassion in dealing with seriously ill patients and their families.
• Reliable transportation.

Qualifications

• Licensure as a Social Worker (LSW) in the state of practice.
• Knowledge of key psychosocial issues prevalent with terminally ill patients and their families.
• Capacity to relate and work well with others as a member of an interdisciplinary team.
