About the role

We are seeking a dynamic and compassionate individual to join our team as a Clinical Social Worker. Your role will involve conducting patient assessments, providing referrals, coordinating discharge plans, and collaborating with the multidisciplinary team to address the comprehensive needs of patients and their families.

Responsibilities

• Conduct patient assessment interviews
• Provide referrals and coordinate discharge plans
• Identify and provide helpful resources to patients and families
• Maintain patient records and communicate critical information to the medical team
• Maintain professional and technical knowledge and required certifications through continuing education and development
• Utilize management resources
• Participate in on-call rotation
• Adapt behavior to the specific patient population

Requirements

• Master's degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE)
• Preferred: Licensed Master Social Worker (LMSW) as verified by the LABSWE preferred or must schedule and pass exam within 6 months of employment
• Must have computer skills and dexterity required for data entry and retrieval of required job information
• Must be proficient with Windows-style applications, keyboard, and various software packages specific to role
• Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
• Working knowledge of common disease entities and disorders and medical terminology in order to understand acute and ongoing needs of patients and families
• Working knowledge of an acute and chronic care medical setting, medical terminology and the psychosocial and economic effects of illness and/or trauma upon the patient and their support system
• Knowledge of health care systems and public programs such as those funded by Medicare, Medicaid, Medicare Advantage, and commercial insurance plans
• Strong interpersonal skills necessary to deal effectively and courteously with patients, families, multidisciplinary team members, departmental peers, and community agency personnel

Qualifications

• Education: Master's degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE)

The schedule for this role will be determined based on the needs of the department and the individual's qualifications. It may include shifts that require flexibility and may include weekends, holidays, and on-call availability.
