About the role
The Division of Infectious Diseases is seeking a Social Worker to expand its growing medical case-management team in the Bluegrass Care Clinic. This position provides medical case management to patients enrolled in the Ryan White Part B program, addresses social needs, and reduces barriers to care so that providers and clinical staff can focus on patient-care issues. The Social Worker Senior is part of a large multidisciplinary healthcare team dedicated to delivering high-quality HIV medical care and a seamless continuum of care to vulnerable people living with HIV in Kentucky. The position is fully funded by Ryan White Part B grant funds issued by the Kentucky Cabinet for Health and Family Services (CHFS) and is located at Bluegrass Care Clinic, 3101 Beaumont Centre Circle, Lexington, KY.
Responsibilities
- Provide medical case management to Ryan White Part B enrollees.
- Identify and mitigate social determinants that impede access to care.
- Collaborate with a multidisciplinary team to ensure continuity of HIV care.
Requirements
- Master of Social Work (MSW) degree.
- Certified Social Worker (CSW) license.
Qualifications
- Proficiency with Windows PC and Microsoft Office (Word, Excel, databases).
- Strong communication, organizational, and problem-solving skills.
- Bilingual (Spanish/English) preferred.
- One or more years of social-work experience, preferably in a medical setting.
- Experience with case management, grant work, clinical settings, healthcare, mental health, or insurance.
- Experience serving people living with HIV/AIDS is preferred.
Physical Requirements
- Prolonged sitting.
- Occasional lifting of objects up to 25 lbs.
- Repetitive motions.
Schedule
- Day shift: Monday–Friday, 8:00 AM–5:00 PM.
Pay
$54,080–$95,056 per year.
