About the role

The New Mexico Veterans Affairs Health Care System (NMVAHCS) is seeking a Social Worker Program Coordinator to lead the planning, development, implementation, and evaluation of social work policies, procedures, goals, and objectives within the Geriatric and Extended Care (GEC) team.

Responsibilities

  • Serves as the sole practitioner in a specialty program and typically provides direct patient care services in the program area.
  • Oversees the daily operation of the program, develops policies and procedures for program operation, prepares reports and statistics, manages the program's budget, monitors staff compliance with practice standards and guidelines on documentation, workload, data entry, ethical practice, and service delivery.
  • Provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service.
  • Coordinates and implements one or more specialty programs across the facility.
  • Develops and monitors social work programs in alignment with the Social Work Service (SWS), directly supervising Program Coordinators and managing these programs.
  • Manages the clinical practice of social workers assigned to the GEC service line, including Community Health Program (CH) Social Worker, Palliative/Hospice Care Coordinator, and Palliative/Hospice Care Social Workers, Home-Based Primary Care (HBPC) Lead and HBPC Social Workers, and Community Living Center Social Worker.
  • Assumes technical and administrative responsibility for the management of social work programming in the Extended Care section of SWS, as well as administrative and clinical supervisory responsibility for social work staff assigned to the GEC programs.
  • Exercises initiative and independent judgment in day-to-day activities, initiating consultation with the SWS Chief and other authoritative resources when necessary.
  • Maintains oversight of the clinical practice of assigned social work staff through the Extended Care Social Work Program Manager.

Requirements

  • Master's degree in social work (MSW) from a CSWE-accredited school.
  • Licensed or certified by a state at the advanced practice level, capable of providing supervision for licensure.

Qualifications

  • One year of experience equivalent to the GS-12 grade level.
  • Advanced practice skills and judgment.
  • Local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
  • Organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Collaborating with internal and external partners to further program goals and enhance patient-centered care.
  • Providing the full range of supervisory duties including assignment of work to be performed, performance evaluations, and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
  • Overseeing the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecasting resource and equipment needs, and administering the allocated budget.
  • Knowledge of organizational structure and impact on program operations.
  • Geriatric and extended care experience, with familiarity in Skilled Nursing, Home Health Care, and Home-Based Primary Care.
  • Supervisory experience preferred.

Pay
