Social Worker PRN
Saint Luke's North Hospital is seeking a PRN Social Worker to join our team. Responsible and accountable for assessing the psychological, social, emotional, cultural, or financial needs that impact a patient’s health and recovery.
About the role
Provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness, crisis intervention, and post-discharge planning. Collaborate with members of the health care team in the development of a transition plan and address needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.
Responsibilities
- Complete family therapy sessions with patients and their families
- Participate in daily treatment team meetings
- Provide psychoeducational group therapy sessions to patients
- Assist with discharge planning, making referrals, and assisting with transportation arrangements
Requirements
- Master's in Social Work required
- LMSW (or LCSW) in MO required
- Applicable experience: Less than 1 year
Schedule
PRN, shifts available any day of the week, hours are 8 a.m. to 4:30 p.m.
