About the role
Join a dedicated clinical team as a Full-Time LPN, responsible for assisting residents with Activities of Daily Living (ADL), promoting independence, and identifying changes in medical and behavioral conditions.
Responsibilities
- Developing, monitoring, and reassessing individualized care plans based on resident needs.
- Providing nursing care aligned with physician’s orders, recognized standards, and policies.
- Coordinating preventative, therapeutic, and rehabilitative services with interdisciplinary teams.
- Reporting on community conditions, personnel performance, resident status, and supply needs to the Director of Health Services.
Qualifications
- Valid nursing license in the state of practice.
- Willingness to respond to emergencies and provide timely care.
- Preferred minimum of one year experience in a skilled facility.
Skills

- Professionalism, attention to detail, and excellent communication skills.
