Social Worker- PRN

About the Role

Tennessee Quality Care Hospice is seeking a licensed PRN Social Worker to join their growing team in Martin. This role provides counseling, resources, and coordination with the clinical team to ensure holistic care for patients and families in the comfort of their homes. The position serves the Martin, TN area and surrounding counties.

Responsibilities

• Perform psychosocial assessments of patients and families/caregivers in the hospice program promptly and ensure identified needs are addressed in the patient’s plan of care.
• Conduct assessments in the patient’s residence and provide assistance when the environment is unsafe, creating an alternative plan as needed.
• Demonstrate exceptional service to patients, families, referral sources, co‑workers, and other customers, and assist team members to maintain this level of service.
• Counsel patients, families/caregivers, and staff regarding stress and other coping difficulties.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assist with applications for Supplemental Security Income, hospice, Medicaid, and Medicaid Waiver.
• Obtain resources from community social, health, pharmaceutical, and welfare agencies to meet patient/family needs.
• Serve as liaison between patients and families/caregivers and community agencies.

Qualifications

• Master’s Degree in Social Work (MSW) – required.
• Current Tennessee licensure as LMSW or LCSW – required.
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance.

Schedule

As needed. Shifts are Monday‑Friday, 8:00 am – 4:30 pm.
