About the role
The Social Worker shares the responsibility of identifying and providing appropriate interventions related to the social and emotional needs of our long-term care residents. Assists with arranging services to meet the needs of residents. Acts as a liaison between the resident, family, facility staff, and the community.
Responsibilities
- Visits resident(s) upon admission to assess the social, emotional and environmental needs of the resident(s) and monitors on an ongoing basis.
- Prepares and maintains required documentation/assessments as regulated by local, state and federal agencies and as problems/situations occur.
- Participates in ongoing evaluation of the residents' needs and goals through resident care plan meetings.
- Monitors adherence to plan of care and flow of information.
- Provides continuing contact with resident/family to coordinate ongoing care.
- Provides crisis intervention, reassessment and uncovers problems as they occur in the delivery of services.
- Provides counseling and emotional support in relation to health/social needs and plan of care.
- Provides ongoing emotional support to residents and families to enable them to cope with difficulties and proper utilization of services.
- Acts as liaison between resident/family and relevant professionals, programs, agencies, etc. to help resident secure services needed and make preferences known.
- Coordinates discharge planning.
- Participates in internal transfers throughout each community as the need arises.
- Coordinates the placement of residents to alternative care facilities when necessary.
Qualifications
- Bachelor's Degree in Social Work or related field
- One or two years previous experience in medical setting, preferably nursing home based
- Skills needed in group process and communications, both verbally and written
- Ability to interact with other professionals in a teamwork manner
