Social Worker - Outpatient LMSW

About the role

Full-time Social Worker position in Troy, NY, providing advanced clinician social work services, care planning, case management, and community resource coordination to patients and their families who utilize inpatient and outpatient medical services.

Responsibilities

• Provide advanced clinician social work services, care planning, case management, and community resource coordination.
• Use motivational interviewing, assessment, and clinical interventions to positively affect the physical, cognitive, and emotional functioning of patients and their families.
• Participate in QI initiatives within the hospital ambulatory setting.
• Collaborate with multi-disciplinary teams to deliver integrated care and ensure appropriate utilization of resources.
• Assist members in Accountable Care Organizations, ensuring high outcomes, satisfaction, and value in total cost of care.
• Mitigate Social Influencers of Health and connect patients to community-based organizations (CBOs), mental health, and substance use disorder resources.

Requirements

• Master’s degree in Social Work.
• NYS LMSW or LCSW preferred.
• Three years of previous clinical experience in a medical, psychiatric, or healthcare setting required.
• Some supervisory experience preferred.

Pay

$30.00–$43.08 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act will be paid within the base salary equivalent of the stated hourly rates. The pay range may vary within the stated range based on location.
