Core Job Responsibilities
- Patient Assessment & Evaluation
- Carry out comprehensive biopsychosocial assessments to identify patient needs, strengths, and challenges.
- Evaluate patients’ mental health, social, and environmental factors impacting their overall well-being.
- Make recommendations based on assessment findings.
- Treatment Planning & Intervention
- Develop and implement individualized treatment plans in collaboration with patients, families, and the healthcare team.
- Provide individual, group, and family therapy sessions to address psychological, emotional, and social issues.
- Support patients in managing chronic conditions, adjusting to life changes, and addressing psychosocial stressors.
- Facilitate crisis intervention and support for patients and families during acute or challenging situations.
- Case Management
- Coordinate and manage care transitions between hospital, skilled nursing, and outpatient settings to ensure continuity of care.
- Aid patients in navigating and accessing community resources and support services.
- Advocate for patients' needs and rights within the healthcare system and broader community.
- Multidisciplinary Collaboration
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure integrated and comprehensive patient care.
- Participate in care team meetings, case discussions, and discharge/care planning processes.
- Documentation & Compliance
- Maintain accurate and timely documentation of patient assessments, treatment plans, progress notes, and other required records.
- Adhere to state and federal regulations, as well as organizational policies and procedures, ensuring compliance in all aspects of social work practice.
- Education & Support
- Provide education to patients and families about coping strategies, mental health conditions, and available resources.
- Offer training and support to staff members on social work practices and psychosocial issues.
- May provide clinical supervision to assigned students, residents, Mental Health Counselors, LMSW or MSW staff within scope of practice.
- Continuous Improvement
- Engage in ongoing professional development and training to stay current with best practices in social work and healthcare.
- Contribute to quality improvement initiatives and program development within the health system.
- Meet CEU requirements to maintain licensure.
- Perform related duties as assigned.
- REQUIRED:
- Master’s Degree in Social Work (MSW) from an accredited institution.
- Minimum of 3 years of clinical experience, with a background in hospital inpatient or outpatient settings.
- Strong understanding of social work practices, ethical standards, and relevant healthcare regulations.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively within a multidisciplinary team and manage a diverse caseload.
- Proficiency with electronic health records and documentation practices.
- Ability to handle stressful situations with professionalism and empathy.
- Ability to travel to other worksites within MVHS.
- PREFERRED:
- Experience with EPIC.
- REQUIRED:
- Current and valid New York State licensure as a Clinical Social Worker (LCSW).
