Social Worker Outpatient

What You Will Do in This Role

• Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support
• Research and advocate for appropriate public assistance resources for clients
• Referral to community resources, assistance with socio-economic problems
• Communicate with client’s care team
• Provide crisis intervention (referral to DCF, provider, etc as needed)
• Cookordination treatment planning with specialists and outpatient providers as needed
• Evaluate current patients for appropriate social work services
• Judge relative costs/benefits of potential social services and health care actions and choose the most appropriate one

Qualifications You Will Need

• EDUCATION: Bachelors in Social Work (required)
• EXPERIENCE: Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse
• Minimum of 2 years prior experience
• CERTIFICATION/LICENSE: State licensure or certification commensurate with degree
