About the Role

The Social Worker MSW on call provides comprehensive biopsychosocial/spiritual assessments, diagnoses, interventions, and advocacy for adult patients. Under supervision, they develop and implement evidence-based, person-centered care plans in collaboration with multidisciplinary teams, respecting patient self-determination and aiming to reduce health disparities and barriers to care. The role involves applying concepts from lifespan development, neurobiology, and behavioral change to support patients and their support systems across the care continuum.

Qualifications

• Master's Degree in Social Work
• Oregon Clinical Social Work Associate License within 60 days of hire

Preferred Experience

• 2 years clinical experience in healthcare, hospice, or bereavement settings
