About the role

The Social Worker provides comprehensive biopsychosocial and spiritual assessments, diagnoses, and interventions for patients and their support systems. Collaborating with patients and multidisciplinary teams, they develop and implement evidence-informed, person-centered care plans that respect patient self-determination, reduce disparities, and align with professional ethical standards. The role involves performing duties in a manner that promotes equitable care and supports vulnerable populations. This role is on-site.

Responsibilities

• Conduct biopsychosocial/spiritual assessments and provide diagnosis and treatment/interventions
• Collaborate with patients and healthcare teams to develop personalized care plans
• Advocate for patients, ensuring respectful, equitable care aligned with ethical standards
• Support patient self-determination and address barriers to care

Qualifications

• Master's Degree in Social Work
• Oregon Clinical Social Work Associate License (to be obtained within 60 days of hire)

Skills

• Preferred: 2 years of clinical experience in healthcare, hospice, or bereavement

Pay

$36.99 to $57.44 per hour
