Social Worker MSW PRN

About the role

As a Social Worker with Research Medical Center, you will join an organization devoted to community volunteering and providing comprehensive patient‑centered care across inpatient floors, outpatient emergency department, and specialty units.

Responsibilities

• Perform non‑clinical biopsychosocial assessments and interventions for inpatient floors and outpatient emergency department.
• Provide crisis intervention, patient/family interventions, high‑risk screening, focused emotional support, and referrals for financial assistance or other resource needs.
• Assist with post‑acute care placement on discharges, focusing on patients with post‑discharge behavioral health and resource needs.
• Respond to physician and non‑discharge planning social work consults throughout the facility.
• Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs.
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual factors.
• Maintain thorough knowledge of Medicare, Medicaid, and private payer regulations and processes.
• Understand managed‑care concepts and length‑of‑stay management relating to patient and organizational fiscal responsibility.
• Perform thorough assessments and develop appropriate discharge plans for high‑risk patients with discharge planning or psychosocial needs.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers.
• Assist patients and families in making difficult decisions while honoring their preferences and values.
• Intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic, acute, or terminal illness.
• Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet emotional, social, and financial aspects of care.
• Provide emotional support to patients and family members during complex and/or emotionally charged issues such as end‑of‑life, terminal illness notifications, death notification (with physician), and crisis interventions.

Qualifications

• Master’s Degree in Social Work (MSW).
• Minimum of one (1) year of hospital Social Work experience preferred.
