About the role
The Social Worker (MSW) at HCA Florida Brandon Hospital is responsible for non-clinical, biopsychosocial assessments and interventions for various departments including intensive care units, Trauma, Oncology, NICU, Transplant, and ED. This role involves collaborating with multiple healthcare professionals to address patient needs, manage discharge planning, and provide emotional support.
Responsibilities
- Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
- Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual issues.
- Maintain a thorough working knowledge of Medicare, Medicaid, and private payer regulations and processes.
- Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
- Coordinate and facilitate meetings between the care team and patients/families/caregivers.
- Assist patients and families in making difficult decisions while honoring their preferences and values.
- Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
- Provide emotional support to patients and family members during complex and/or emotionally charged issues, such as end-of-life, terminal illness notifications, death notification (with physician), and crisis interventions.
Qualifications
- Average of 2+ years experience in case management OR 3+ years experience in clinical nursing required.
- Ace or Nursing Diploma required. Bachelor's Degree in Nursing preferred.
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation required.
- InterQual experience preferred.
- Certification in Case Management preferred.
- Ability to establish and maintain collaborative and effective working relationships.
- Communicate effectively in oral, written, and electronic formats.
- Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills.
