This is a PRN position to help cover both weekdays and weekend shifts as needed. Hours are 8:00am - 4:30pm. Training/orientation will take place during weekdays for the first few weeks.

About the role

The Social Worker (MSW) is responsible for non-clinical, biopsychosocial assessments and interventions for both inpatient floors and the outpatient emergency department. The role includes crisis intervention, patient/family interventions, high-risk screening, focused emotional supports, and referrals for financial assistance or other identified resource needs. The MSW assists in post-acute care placement for discharges, particularly for patients with post-discharge behavioral health and resource needs, and facilitates Family Care Conferences to support patient, family, and discharge planning goals. The MSW is dedicated to patient and family-centered care, valuing personal self-determination, prioritization, and compassionate problem-solving.

The MSW maintains a thorough working knowledge of Medicare, Medicaid, and private payer regulations, as well as managed-care concepts and length-of-stay management. The role covers intensive care units, Trauma, Oncology, NICU, Transplant, ED, and responds to physician and non-discharge planning social work consults throughout the facility.

Responsibilities

  • Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs.
  • Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual challenges.
  • Maintain a thorough working knowledge of Medicare, Medicaid, and private payer regulations and processes.
  • Understand managed-care concepts and length-of-stay management relating to patient and organizational fiscal responsibility.
  • Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
  • Coordinate and facilitate meetings between the care team and patients/families/caregivers.
  • Assist patients and families in making difficult decisions while honoring their preferences and values.
  • Intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic, acute, or terminal illness.
  • Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet emotional, social, and financial aspects of care.
  • Provide emotional support to patients and family members during complex or emotionally charged issues, such as end-of-life care, terminal illness notifications, death notifications (with physician), and crisis interventions.

Requirements

  • Master’s Degree in Social Work required.
  • Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker/Registered Clinical Social Worker-Intern (LMSW/RCSW-I) required.
  • Certification in Social Work preferred.
  • Licensed Clinical Social Worker (LCSW) preferred.
  • Minimum of 1+ years of medical social work experience.
