Summary

The HUP Inpatient Medicine Department is looking for a full‑time Social Worker to join the team. The Social Worker will conduct bio‑psychosocial assessments, provide support to patients and families, develop safe and timely discharge plans, and facilitate access to community resources and support services. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

Responsibilities

• Coordinate the discharge planning process to ensure a safe and timely transition.
• Collaborate with the interdisciplinary health‑care team to ensure continuity of care and support services to patients and families.
• Provide support to patients and families.
• Document all activities in the medical record.
• Maintain relationships with community agencies and stay up‑to‑date on resources.
• Use effective interpersonal skills in all interactions.
• Enhance professional development and pursue formal and informal continuing education for self‑improvement.

Education

Master’s in Social Work (required).
