About the role
The Social Worker plays a crucial role in the interdisciplinary team, providing and overseeing psychosocial care for selected patients and families. They conduct bio-psychosocial assessments, provide intervention and treatment as needed, and collaborate with other healthcare professionals.
Responsibilities
- Identify and refer for specialized services such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
- Assess and intervene in domestic violence and child abuse situations.
- Participate in case reviews to evaluate case management and progress.
- Consult with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
- Develop comprehensive bio-psychosocial assessments tailored to the patient's age and cultural needs.
- Employ a range of clinical interventions such as adjustment to illness counseling, crisis intervention, care coordination, and information and referral.
- Advocate for patients and families to gain access to services and resources.
- Support patients with mental health needs during the episode of care and discharge planning.
- Collaborate with Memorial Healthcare System SDOH Hub and other community resources.
- Respond to requests for assistance with Social Determinants of Health and advocate for support in the community.
Requirements
- Masters: Social Work (Required)
- A minimum of one (1) year of social work experience in a health care setting.
Qualifications
- Must display a high level of flexibility, adaptability, and organizational skills.
- Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
- Communicates effectively and respectfully with patients, families, and all levels of staff and management.
- Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns.
- Manages crisis situations, demonstrating ability to quickly appraise and select appropriate intervention.
- Demonstrates proficiency in using the electronic medical record.
- Displays good problem-solving skills, root-cause analysis, and principles of clinical performance improvement.
