About the role
The Social Worker plays a crucial role in the interdisciplinary team, providing and overseeing psychosocial care for selected patients and families. They conduct bio-psychosocial assessments, provide intervention and treatment as needed, and collaborate with other healthcare professionals.
Responsibilities
- Identify and refer patients to specialized services like drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
- Support patients with mental health needs during the episode of care, discharge planning to both inpatient and outpatient mental health providers as applicable.
- Collaborate with and provide social work consultation to other disciplines within the healthcare setting and the community.
- Respond to requests for assistance with Social Determinants of Health and advocate for support in the community.
- Refers to and collaborates with Memorial Healthcare System SDOH Hub.
- Afford psychosocial support and interventions, particularly for individuals with chronic and/or complex medical and behavioral health conditions.
- Supports the department's initiatives, mission, and goals and may take on additional responsibilities at the discretion of leadership.
- Performs all aspects of psychosocial discharge planning for chronic conditions and/or long-term custodial care needs for inpatient discharge, community services, outpatient care and any other applicable setting.
- Provides assistance with entitlements, medications, transportation, and advance directives.
- Develops comprehensive bio-psychosocial assessments responsive to age-appropriate and cultural needs and concerns.
- Employs a range of clinical interventions such as adjustment to illness counseling, crisis intervention, care coordination, and information and referral and safety planning.
- Advocates on behalf of patients and families to gain access to services and resources.
- Participates in case reviews to evaluate case management and progress.
- Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
- Provides assessment and intervention in domestic violence and child abuse situations.
Requirements
- Masters: Social Work (Required)
- One (1) year social work experience in a health care setting.
Qualifications
- Must display a high level of flexibility, adaptability and organizational skills.
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to prioritize and organize time and functions.
- Must communicate effectively and respectfully with patients, families, and all levels of staff and management.
- Must be able to recognize and manage risks, resolve issues and problems as soon as they arise, report and document adverse events and reportable conditions.

- Must demonstrate the ability to provide social work services to patient, family members and significant others in four areas: psychological functioning, adaptation to illness, injury, disability or life event, psychosocial assessment/evaluation and facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.
- Must be able to identify and intervene for suspected abuse including sensitivity to age-specific concerns; physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; and caretaker/institutional abuse for senior adults.
- Must be able to manage crisis situations, demonstrating ability to quickly appraise the crisis situation and select appropriate intervention.
- Must be able to document direct service in patient’s medical record after each significant encounter.
- Must demonstrate proficiency in use of the electronic medical record.
- Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement.
