Social Worker, MSW

We help New Yorkers of all ages across all five boroughs with highly individualized treatment programs that address psychological, physical, social, family, educational, and spiritual needs to support personal rehabilitation, renewal, and family restoration.
About the role
The Social Worker position requires a wide range of clinical skills including interviewing, individual and group counseling, and a thorough knowledge of substance abuse, mental health, and human service system resources. The Social Worker is responsible for the assessment and treatment of clients with co-occurring mental health issues, serving as Health Coordinator for communicable disease education and referral, and functioning as part of a Multidisciplinary team. The role includes reviewing treatment plans as a Qualified Health Professional, participating in Quality Assurance, and ensuring regulatory, billing, and utilization review compliance.
Responsibilities
- Complete clinical assessments and re-assessments at regular intervals to measure progress toward addressing life area deficiencies.
- Provide individual counseling to address outstanding family, medical, psychiatric, and housing concerns.
- Collaborate with each consumer to formulate, monitor, review, and update treatment plans in accordance with regulatory requirements and clinical needs.
- Conduct evidence-based group counseling services related to a wide range of clinical and community issues.
- Maintain up-to-date resident charts including assessments, treatment plans, case notes, and documentation of collateral contacts.
- Provide crisis intervention services.
- Assist in the development and implementation of consumer discharge plans.
- Work closely with utilization review staff to ensure completion and accuracy of all necessary health insurance authorization documentation.
- Establish community and agency affiliations to address consumer needs and strengthen service delivery.
- Function as liaison and provide education and direct services to family members regarding consumer’s substance use and related behavioral issues.
- Maintain a caseload when necessary due to staff absences or vacancies.
- Conduct consumer screening for contraband and collect samples for urine toxicology testing when necessary.
- Attend regularly scheduled staff meetings and case conferences.
- Facilitate case conferences and conduct internal chart audits when necessary.
- Attend all required in-service training seminars.
- Participate in quality improvement activities.
- Function as “Manager on Duty” in the absence of an on-site manager.
- Perform other relevant duties as required.
Requirements
- Master’s Degree in Social Work, Counseling, or Marriage and Family Therapy required.
- One (1) year of experience working in a social service setting providing group counseling, family therapy, and case management with chemically dependent clients preferred.
- NYS Licensure as a Social Worker, Mental Health Counselor, or Marriage and Family Therapist preferred. Individuals obtaining a limited permit will be considered.
- Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook programs).

Pay
Salary Range: $58,500.00 - $70,000.00
