Social Worker MSW
Job Description
This position is responsible for assessing psychosocial needs of patients/families/significant others; providing professional counseling, case management, and discharge planning services as indicated. Services are provided in accordance with professional standards and in consultation and/or collaboration with the multi-disciplinary health care team, as warranted, to assist/encourage patients/families/ significant others to utilize medical care constructively to assure the achievement of an optimal level of functioning in all patient care settings in the community, as well as alternate levels of care.
Education And Credentials
- Masters Social Work or related field required.
- NYS Drivers License upon hire required.
- PASRR Screen within 6 Months required.
- LMSW, LCSW or LCSWR required for Oishei Childrens Hospital Psychiatry Clinic and School Health Services at OCH (must be active and maintained).
- NYS DOH PASRR Screen Certification is required (regulatory) within six months of hire/transfer for all inpatient departments.
- If not offered in 6 months, must take the next available class.
Experience
- 3 years of experience in Discharge planning in an acute care or nursing home setting for inpatient departments only, inclusive of transitioning patients from the acute care or nursing home setting, educating patients and families on placement process and understanding services available in the community and the referral process.
- 3 years of experience in counseling required, excluding School Health Services departments.
- Inpatient departments: 3 years of experience in discharge planning experience in an acute care or nursing home setting. Inpatient departments: Nursing home knowledge preferred.
- School Health Services only: 1 year of experience in Child and adolescent mental health counseling specifically working with school aged children and related mental health concerns required.
