About the role

Social Workers help communities access the health or social services they need, improving or providing those services where they are not already in place. They participate in legislation and help families and individuals access counseling and psychotherapy when necessary. Social Workers are essential advocates who work on behalf of those facing physical and mental illness, disability, divorce, loss, unemployment, abuse, addiction, and other daunting life challenges.

In this role, you will complete comprehensive, culturally competent assessments including consideration of biomedical, psychosocial, and spiritual factors of the patient and family's needs, leading to the implementation of appropriate social service interventions and continuing care options.

Responsibilities

  • Demonstrate the knowledge and ability to care for age-specific needs of the population served as measured by completion of mandatory training and direct observation by supervisor.
  • Demonstrate an understanding of illness/recovery and the impact on patients and families.
  • Respond to campus-wide referrals on the same working day.
  • Identify significant psychosocial issues and implement appropriate social service interventions and continuing care options through data collection, analysis, and implementation of a care plan based on identified needs.
  • Utilize a multidisciplinary approach to ensure continuity of care and comprehensive patient services to maximize the effectiveness of social service interventions.
  • Develop and sustain positive working relationships with patients, families, physicians, nurses, and ancillary personnel to promote open communications.
  • Act as a resource to patients, families, physicians, and caregivers regarding internal/external resources.
  • Initiate team conferences to facilitate discussion/consensus regarding complex cases.
  • Use crisis intervention techniques to calm, comfort, and stabilize patients/families under stress.
  • Coordinate realistic referrals and continuing care plans to meet the needs, age, and health status of the patient.
  • Inform patients, families, and anyone accountable for the continuing care of the patient of their role and responsibilities relating to the continuing care plan.
  • Utilize appropriate clinical intervention skills to assist patients, families, and caregivers with their abilities to cope based on assessed needs.
  • Document on CM progress notes, multidisciplinary problem list, and interdisciplinary education records to provide a clear, concise record of social work services and interventions.
  • Facilitate/coordinate timely transfer and discharge of patients with complex needs.
  • Facilitate/coordinate inpatient and outpatient dialysis arrangements.
  • Disclose medical errors to patients and families in accordance with the existing patient safety plan and disclosure policy, in collaboration with clinical and medical staff.
  • Assist in developing and maintaining the electronic case management resource manual.
  • Attend to represent Cleveland Clinic at the Broward County Baker Act Task Force meeting.

Requirements

  • Master’s Degree in Social Work.
  • One year of clinical experience.
  • Basic computer skills with familiarity with Windows-based software programs.
  • Experience in crisis intervention, psychosocial, and therapeutic counseling.
  • Experience in utilization management, managed care, quality, and payor issues.
  • Strong organizational and prioritization skills and demonstrated leadership skills.

Preferred Qualifications

  • State Licensed Clinical Social Worker (LCSW).
  • Two years of experience in hospital/healthcare Social Work.
  • Knowledge of acute care regulatory/accreditation requirements.

Physical Requirements

  • Requires frequent walking from department to department.
  • Requires sitting at a workstation or desk.
  • Requires standing.
  • Work may include occasional pushing and/or pulling, lifting, and carrying objects weighing up to 20 pounds, such as files, documents, and computer printouts.
  • Work requires finger dexterity and eye/hand coordination to operate a computer keyboard at a moderate skill level.

Personal Protective Equipment

Follows Standard Precautions using personal protective equipment as required for procedures.

Schedule

Works days from 8:00 AM – 4:30 PM, with weekend and holiday requirements.
