Responsibilities
- Providing clinical and case management services in community-based settings, including homes, schools, and other off-site locations.
- Developing, implementing, and updating individualized treatment plans in collaboration with clients and families.
- Maintaining timely and accurate clinical documentation and required agency reports (e.g., clinician schedules, progress notes, treatment plans).
- Monitoring treatment goals and updating treatment plans on a regular basis.
- Collaborating with community partners, schools, and other service providers to ensure coordinated care.
- Providing crisis intervention and support as needed.
Requirements
- Master’s Degree in Social Work or related clinical field.
- Eligible for or in possession of NYS licensure (LMSW/LCSW/LMHC).
- Ability to travel to different sites in the community; Access to a vehicle and valid driver’s license.
- Experience working with youth with mental health needs.
- Bilingual English/Spanish preferred.
- Strong communication skills and ability to work effectively with culturally diverse populations.
- Collaborative and adaptable professional who values continuous learning and teamwork.

Pay
$60,000 - $85,000, determined by licensure, plus a $5,000 differential for bilingual (English/Spanish) fluency.
