Social Worker - MHCAH (U) (PV)

About the role

To provide skilled, cost-effective social work care in a competent holistic manner for a designated group of patients, through assessment, planning, implementation and evaluation of social work care, to empower patients and families to achieve maximum levels of wellness and independence.

Responsibilities

• Assess patient needs and develop appropriate care plans
• Implement and monitor social work interventions
• Evaluate the effectiveness of social work care
• Collaborate with interdisciplinary teams to enhance patient care
• Provide counseling and support services to patients and families

Requirements

• Graduate of a Masters Program in Social Work
• One of the following certifications: LCSW, LMSW, or LSW
• Experience in a clinical setting preferred

Qualifications

• Excellent interpersonal and communication skills
• Strong problem-solving abilities
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite

Skills

• Knowledge of social work principles and practices
• Ability to manage caseloads effectively
• Strong organizational and time management skills
