Social Worker LSW / CSW Per Diem

Responsibilities

• Affords assistance to residents with emotional adjustment to the facility.
• Interviews new residents and documents their social history and assessments, as directed.
• Completes the social services portion of the MDS, care planning, baseline, and other relevant tasks.
• Refers residents to psychological services when necessary.
• Schedules and attends interdisciplinary care plan meetings, morning meetings, utilization reviews, and other related sessions.
• Writes summaries and progress notes as required.
• Conducts pre-discharge evaluations, including coordinating with visiting nursing services and community resources.
• Assists with discharge planning and follows up after discharge.
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family members.
• Educates residents and representatives on advanced directives.
• Adheres to all Social Services Regulations.

Qualifications

• Minimum Bachelor's Degree in Social Work from an accredited college.
• Must obtain a CSW and/or LSW license.
• One to three years of professional experience and/or training, or equivalent combination of education and experience.
