Core Responsibilities

• Conduct psychosocial assessments
• Collaborate with the multidisciplinary team
• Provide palliative education and support
• Coordinate care and discharge planning
• Facilitate communication between patient and team
• Design and provide education on palliative care
• Offer ongoing psychotherapeutic intervention and counseling

What You Will Do

• Conduct psychosocial assessments
• Assess physical, psychological, socioeconomic, spiritual, and symptom management needs
• Assess advance care planning needs
• Develop a social work treatment plan
• Communicate with the outpatient and inpatient palliative care teams
• Collaborate with community hospices, home care agencies, and long-term care facilities
• Participate in palliative care quality improvement initiatives
• Facilitate family meetings
• Build respect and trust within the team
• Seek out and enhance job-related knowledge through local, state, and national palliative care initiatives

Education Qualifications

• Master's Degree in Social Work (MSW) preferred

Experience Qualifications

• Minimum of one year post-Master's experience in a healthcare setting preferred

Knowledge, Skills, And Abilities

• Advanced social work assessment skills
• Effective communication
• Critical thinking skills
• Ability to collaborate as part of a multidisciplinary team

Licenses and Certifications

• Licensed Social Worker upon hire required
• LCSW upon hire preferred

Pay Range

$72,800.00 - $87,360.00
