Social Worker - Full Time (LMSW, LCSW)

About the role

Outpatient Social Workers are assigned to work a territory within our coverage area of NWA and NE Oklahoma. The Social Worker is an integral part of the Interdisciplinary team, responsible for the development and oversight of a plan of care for an assigned group of patients'/families' psychosocial, financial, and emotional needs. Provides coverage for the inpatient and/or outpatient Hospice program as needed. Liaisons with community resources to find support for the patient/families as needed. Flexibility with schedule to accommodate the patient and organization needs.

Responsibilities

• Develops and oversees a plan of care for assigned patients'/families' psychosocial, financial, and emotional needs.
• Provides coverage for the inpatient and/or outpatient Hospice program as needed.
• Liaises with community resources to find support for the patient/families as needed.
• Works independently and maintains good working relationships with patients, families and coworkers.
• Mentors interns and other students.
• Facilitates bereavement programs as needed.

Requirements

• Current Arkansas Social Work license without restriction.
• Access to a working vehicle and the ability to drive for extended periods of time.
• Valid Arkansas driver's license and Liability Insurance as required by law.
• Satisfactory driving record.
• Emotionally mature, objective, sensitive to people and their problems, able to handle responsibility, work independently, and maintain good working relationships with patients, families and coworkers.
• Bilingual abilities a plus.
• Excellent computer, phone, reading, writing, and communicating skills required.
• Ability to organize information, time and workload to optimize efficiency.
• Ability to work outside normal office hours, including call coverage, and to handle emergencies as required in order to meet needs of patients and families.
• Good working knowledge of the Social Work scope of practice and regulations.
• Satisfactory results of a current TB skin test, annual TB fit test, and annual flu vaccine required.
• Knowledge of dynamics of death and dying, bereavement and end of life issues preferred.
• Experience in a hospice or healthcare environment preferred.
