Social Worker (LMSW) - PRN

About the role

Crescent Hospice is seeking a patient-centered Licensed Master Social Worker (LMSW) to join our team. The ideal candidate will deliver exceptional social work services to hospice patients and their families.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Provide individualized care advocacy and implement personalized social work plans.
• Collaborate with the Bereavement Coordinator and attend interdisciplinary meetings.
• Address psychosocial stresses and shape care plans.
• Provide empathetic counseling to patients and families.

Qualifications

• Education: Master of Social Work from an accredited school of social work.
• Licenses: Licensed as a Social Worker from the State Board of Social Work; current unencumbered license required.
• Experience: Minimum one (1) year experience as a social worker in a hospice setting; minimum one (1) year experience as a social worker in a healthcare setting; long-term care or medical surgical/acute care setting preferred.
• Additional State-Specific Requirements: Current unencumbered license as a Social Worker from the State Board of Social Work required for AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX.

Pay

$30.00 - $33.00 per hour (Average Pay Range). Final compensation rate will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position.
