About the role
Works as part of an interdisciplinary team to facilitate the delivery of services to patients/families. Coordinates the discharge planning process. Coordinates individual, family, and group counseling sessions. Uses assessment skills to accurately identify patients’ needs and demonstrates the ability to provide a safe, therapeutic environment for all ages, pediatric, youth, adolescent, adult, and geriatric patients. Participates in patient/family teaching. Uses teaching literature and techniques for different age groups and learning needs.
Responsibilities
- Facilitates the delivery of services to patients/families.
- Captures and coordinates the discharge planning process.
- Counsels individuals, families, and groups.
- Identifies patients' needs through assessment and creates a safe, therapeutic environment.
- Teaches patients/families using appropriate literature and techniques.
- Collaborates with interdisciplinary teams to ensure comprehensive care.
- Coordinates with other departments to ensure seamless patient care.
Requirements
- Master's Degree in Social Work.
- Licensure: LMSW required. LCSW preferred.
- Previous experience as an LMSW in a hospital setting strongly preferred.
- Experience with psychiatric patients preferred.
- Clear communication with diverse populations of patients/family units of all ages, variety of cultures, etc.
- Good organizational, prioritization, and problem-solving skills.
- Strong interpersonal skills to establish and maintain effective working relationships.
- Ability to interpret and implement policies and procedures of SGMC Health, regulatory agencies’ standards and rules, reimbursement rules, etc.
- Flexible in work schedule.
- Computer skills required.
Qualifications
