About the Role

To work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. Interventions may include alternate site coordination, discharge planning, addressing abuse/neglect, adoption, substance use disorders, end-of-life decisions, referral resources, bereavement support, adjustment counseling, guardianships, and behavioral health concerns (including psychiatric transfers).

Responsibilities

• Conducts psychosocial assessments with patients and/or families to address identified needs or alternate site of care post-discharge and executes a plan.
• Communicates with alternate sites of care and/or community agencies to address patient needs effectively and timely.
• Completes and documents assessments and plans in the electronic medical record, reflecting the patient’s condition, situational factors, transition of care needs, and psychosocial imperatives.
• Provides consultation and resources to the healthcare team.
• Maintains current knowledge of community resources.
• Participates in department, regional, or statewide stakeholder committees and projects as assigned.
• Develops and maintains knowledge of federal and state regulations relevant to the role.
• Provides short-term therapeutic support as appropriate for the setting and location.

Qualifications

Required

• Master’s Degree in Social Work
• 2 years of healthcare-related experience post-Master’s Degree

Preferred

• Care management experience
• Experience in individual/family assessment, crisis intervention, grief/loss counseling, and discharge planning
• Master Social Worker (MSW) license in the State of Michigan upon hire
