About the role
This position covers Benign Heme, Sickle Cell, and Thalassemia populations. The Social Worker-LMSW will collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs.
Responsibilities
- Work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs.
- Conduct psychosocial assessments with patients and/or families to identify psychosocial needs or to determine if an alternate site of care is needed post-discharge.
- Develop and execute a plan based on the assessment.
- Communicate with alternate sites of care and/or community agencies to address the patient’s needs effectively and in a timely manner.
- Complete and document the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, transition of care needs, and psychosocial imperatives.
- Provide consultation and resources to members of the healthcare team.
- Maintain relevant and current knowledge of community resources.
- Participate or assist in department, regional, or statewide stakeholder committees, projects, etc., as assigned or requested.
- Develop and maintain current knowledge of federal and state regulations as they pertain to the role.
- Provide short-term therapeutic support as appropriate for the setting and location.
Qualifications
- Master's Degree in Social Work
- 2 years of relevant experience in healthcare-related settings post-obtaining the Master's Degree in Social Work
- Care management experience preferred
- Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred
- Preferred: Master Social Worker (MSW-Master) - State of Michigan
