Job Description

The Social Worker LCSW reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

Responsibilities

  • Supports the Northwestern Medicine Hispanic Transplant Program for patients who prefer to communicate in Spanish.

  • Assists patients in navigating complex financial/insurance issues.

  • Provides supportive counseling to patients in their re-adjustment to pre and post-transplant lifestyle and management of complications.

  • Completes timely documentation of all activities in the medical record and hospital-wide information systems.

Qualifications

  • Licensure in Illinois; Licensed Clinical Social Worker (LCSW).
    • Licensure in Illinois; Licensed Clinical Social Worker (LCSW).
  • Master's Degree in Social Work from a school of social work accredited by CSWE.

  • Minimum of two years post-graduate experience in hospital Social Work or related settings.

  • A high level of interpersonal skills to affect positive outcomes.

  • Organizational skills necessary to prioritize and manage an appropriate caseload of patients coupled with performing the Social Worker functions.

Internal employees and rehires who left Northwestern Medicine within 1 year are not eligible for the sign on bonus. Exception: New graduate internal employees seeking their first licensed clinical position at NM may be eligible depending upon the job family.
