Responsibilities
- Pychosocial Assessment & Intervention
- Collaborates with interdisciplinary teams
- Provides crisis intervention and protective services
- Facilitates discharge planning and resource utilization
- Assists in identifying and managing high-risk patients
- Participates in clinical performance improvement activities
Qualifications
- Licensed Clinical Social Worker (LCSW)
- Masters Degree in Social Work from an accredited school
- Two years post-graduate experience in hospital Social Work or related settings preferred
