Social Worker LCSW

Deliver emotional and psychosocial support, along with education, to patients, families, and vulnerable populations. Provide therapy, counseling, care coordination, discharge planning, and advocacy. Work collaboratively within a multidisciplinary team, practicing at the highest level of licensure, applying evidence-based approaches, and demonstrating flexibility across individual, group, and crisis interventions.

Responsibilities

  • Complete patient assessment, reassessment, and treatment plans as clinically indicated.
  • Assess risk and safety factors and recommend next steps in treatment.
  • Provide clinical interventions including counseling, individual sessions, groups, bereavement, and crisis intervention, as well as assessment and intervention in cases involving family/patient dynamics.
  • Complete required documentation and paperwork related to compliance and regulatory guidelines.
  • Work collaboratively with colleagues, fostering positive relationships within and beyond the team.
  • Approach challenges constructively and professionally, partnering with psychiatric staff and other health professionals to ensure high-quality diagnosis and treatment.
  • Establish objectives and develop patient-focused treatment plans to provide disposition and/or referral.
  • Identify potential barriers, limitations, lack of coping abilities, adjustments, and functional status.
  • Maintain knowledge of available community resources, evaluate for abuse concerns across populations, and ensure timely referrals.
  • Maintain, utilize, and apply evidence-based therapeutic practices and treatment modalities.
  • Contact and communicate with insurers, including managed care companies, to clinically justify treatment and negotiate approval for clinical services.
  • Attend required department, patient, and family meetings.
  • Maintain and model organizational health values.
  • Demonstrate regular, reliable, and predictable attendance.
  • Perform other duties as required.

Requirements

  • Licensed Master Social Worker (LMSW), Licensed Professional Counselor (LPC), or Associate (LPCA).
  • At least one year of demonstrated clinical background and hospital or crisis experience; relevant internship experience may be considered.
  • Preferred: Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
  • Required to submit licensure application materials for LCSW or LPC within two years of hire and successfully pass the examination within six months of eligibility.

Pay

$36.53 - $67.90 hourly
