Social Worker II PRN
Located in Largo in the heart of Prince George’s County, our new state-of-the-art regional medical center (UM Capital Region Medical Center) will provide improved access to primary and ambulatory care services and serve as a tertiary care center for critically ill patients. In addition, our new space will allow us to expand our offerings as a community partner to help improve the health status of Prince George’s County residents.
About the role
The Social Worker II assesses, develops, implements and coordinates care plans for the psycho-social needs of each resident, including supportive counseling to patients and family members. Collaborates with other staff concerning patient admissions and discharges from the treatment facility. Work is performed under the supervision of a Licensed Social Worker.
Responsibilities
- Assesses patients, evaluates psycho-social problems, prepares and implements service care plans.
- Collaborates with Admissions and Patient Accounts in admission and discharge planning.
- Provides ongoing service planning and implements patient rehabilitation plans.
- Assists in the coordination of patient discharges with healthcare providers/agencies for support in the patient's home.
- Supports the Business office in processing all benefits, enrollments, applications, and re-certifications to ensure continual funding.
- Provides family counseling to help explore and resolve concerns regarding the family member residing at the Center.
- Provides ongoing community awareness of services, including distributing information to feeder agencies and at public events.
- Performs departmental administrative duties and attends management team meetings when scheduled.
- Completes information for monitoring quarterly and annual utilization review and other reports.
- Performs related work as assigned.
Requirements
- Master’s Degree in Social Work required.
- Valid Maryland Driver’s License.
- One year of experience in a related social service role required; two years preferred.
- Completion of work study/internship in providing social work and case management services.
Qualifications
Highly Preferred: One or more years of case management experience in a medical provider setting.
Skills
- Proficiency in Microsoft Office Suite.
Pay
Pay Range: $33.40 - $50.13 per hour.
