About the role
Crouse’s Care Coordination Services team is hiring a Social Worker.
Responsibilities
- Works collaboratively with the Care Managers and other members of the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.
- Participates in the development and implementation of a comprehensive integrated discharge plan.
- Identifies patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge.
- Actively participates in the discharge process following federal and state regulations and accreditation standards, managing care across the continuum to ensure a safe discharge within a timely manner.
- Provides crisis interventions to patients and families as needed.
Requirements
- Master’s Degree in Social Work required.
- LMSW license preferred.
- 2 to 3 years of Social Work experience required.
- Case Management, Discharge Planning, or home care experience preferred.
- Excellent oral and written communication and interpersonal skills.
- Demonstrates age-appropriate communication in assigned work area.
- Ability to interact with physicians, members of the multidisciplinary healthcare team, patients, families, and community agencies to achieve optimal patient outcomes.
- Knowledge of federal and state regulations (DOH, Medicare/Medicaid).
- Strong organizational and problem-solving skills.
- Knowledge of third-party payers and/or managed care principles.
Pay
$60,000.00 - $65,905.00 per year.
Schedule
Full-time, day shift, 8:00 AM – 4:30 PM.
