Responsibilities

• Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health to establish a plan of care that fosters personal worth, spiritual well-being, and human dignity.
• Participate as a member of the interdisciplinary team and in the development and review of the plan of care for assigned patients.
• Prepare the patient to deal with changes and the family to support the patient, including education on Advanced Directives.
• Utilize all available resources, such as family, hospice, and community agencies, to assist the patient and family in living better within the limitations of the illness.
• Support the bereavement program at the site level.

Requirements

• Minimum of a Bachelor of Social Work (BSW) degree from a school of social work accredited by the Council on Social Work Education; Master of Social Work (MSW) preferred.
• Successful completion of an approved undergraduate (BSW) or graduate (MSW) level hospice internship.
• Meets state-specific requirements above minimum education standards.
• At least one (1) year of experience in a health care setting.
• Medicare/Medicaid/Insurance specialty preferred.

Pay

$50,000 - $75,000 annually, based on years of experience.
