Social Worker , Hospice MSW

About the Role

Full‑time Inpatient Hospice Clinical Social Worker located in Naperville, IL (on‑site, not remote). You will join AccentCare’s purpose‑driven, collaborative culture, working with a team that provides exceptional care to patients and their families.

Qualifications

• Master’s degree from an accredited school of Social Work or related field.
• Valid license/registration/permit to practice as a clinical social worker in the state of agency operation, if required.
• Medicare/Medicaid/Insurance specialty preferred.
• At least one (1) year of professional social work experience (post‑graduate) in a healthcare setting; 2+ years preferred.
• Experience or education in grief counseling preferred.
• Discharge planning experience preferred.
• Must be a licensed driver able to travel to all business locations.

Responsibilities

• Assist core members of the pre‑hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient’s health.
• Participate as a member of the interdisciplinary team and contribute to the development and review of each patient’s plan of care.
• Assess social, spiritual, and emotional needs of patients and caregivers to gauge capacity and coping ability.
• Prepare patients and chosen families to cope with changes, including education on advance directives and advanced care planning.
• Utilize available resources—family, hospice, community agencies—to help patients and families live better within illness limitations.
• Support the bereavement program.
• Provide discharge planning related to changes in level of care or community placement.

Pay

$65,000 – $85,000

Schedule

Full‑time, Monday – Friday plus on‑call rotation.
