SOCIAL WORKER/DISCHARGE PLANNER, IDH

About the role

Aid patients, their families, or significant others in addressing social, economic, or environmental issues arising from or related to their behavioral health diagnosis. Utilize creative communication strategies to support patients through their healthcare journey. Collaborate with a multidisciplinary behavioral health team including patients, families, physicians, nursing staff, and other healthcare professionals.

Responsibilities

• Aid patients in understanding their diagnosis and coordinating their treatment plans.
• Seek assistance from various social services on behalf of the patient.
• Communicate the patient’s treatment plan to their family and caregivers.
• Document all interactions and outcomes.
• Partner with a network of community agencies to address the patient’s psychosocial needs.

Requirements

• Bachelor's Degree required. Bachelor's Degree in Social Work preferred.
• License or certification in Social Work preferred. MSW or Master in Counseling preferred.
• At least one year of relevant behavioral health social work experience.
• At least one year of experience in counseling.
• Experience in Patient Navigation and Discharge Planning preferred.
• Understanding of social work principles and practices.
• Knowledge of community social and health agencies.
• Knowledge of human behavior and personality dynamics.

Qualifications

• Bachelor's Degree required.
• At least one year of relevant behavioral health social work experience.

• Excellent communication skills.
• Strong organizational and problem-solving abilities.
• Ability to work collaboratively with a multidisciplinary team.
• Knowledge of community resources and social services.
