About the role
Hewitt Nursing and Rehab is seeking an experienced and dedicated Licensed Social Worker to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.
Responsibilities
- Resident, family, and physician intervention
- Psychosocial services
- Vision, hearing, and dental services coordination
- Assessments and care planning
- Discharge planning
Requirements
- 4-year degree in Social Work
- Licensed Social Worker in the State of Texas
- At least 2 years of experience in a Skilled Nursing Facility or Hospital (preferred)
