Essential Duties And Responsibilities
- Advocate for clients to resolve challenges
- Collaborate with other professionals to evaluate client’s medical or physical conditions and assess client’s needs
- Refer client or family to community resources to assist in recovery from mental and physical illness and to provide access to services such as financial assistance, legal aide, housing, education, Benefits, Entitlement and job placement
- Counsel clients in individual and group sessions to help them improve quality of life
- Organize support groups on topics such as anxiety, bereavement, depression, etc.
- Organize support groups or counsel for family members to assist in understanding and supporting clients
- Maintain written records of contact with clients, including case files, worker logs, and computerized client databases
- Attend training as required, including mandatory funding organizations
- Assist agency in outreach activities in community

- Demonstrated experience developing and delivering comprehensive assistance to clients
- Articulated knowledge of health, mental health and wellness issues
- Demonstrated application of evidence-based and evidence-informed health practices
- Demonstrated understanding of and stated commitment to diversity, inclusion, and equity issues
- Demonstrated experience working with diverse populations
- Demonstrated ability to facilitate presentations to large and small audiences
- Demonstrated ability to communicate information effectively
- Demonstrated interpersonal skills necessary for effective work relationships
EDUCATION And EXPERIENCE
- Master’s degree in Social Work
- At least two years’ experience in providing social services required
- Experience in providing case management or social services within the senior population required
