Social Services Worker

Responsibilities

• Captures all admissions to the facility, including re-admissions, room moves, and patient inquiries.
• Coordinates all discharges, writing discharge plans that involve family and community services as needed.
• Might be responsible for coordinating resident and family member grievances and acting as the resident advocate when necessary.

Requirements

• Bachelor’s Degree in social work required.
• Current Social Worker License from the State of practice.
• At least one year of supervised experience in a healthcare setting, preferably in a long-term care facility.
• Current knowledge of state and federal laws applicable to social work in long-term care.
• Excellent interpersonal and communication skills.
