Social Worker-Community Support Program

About the Role

This position provides service facilitation and crisis intervention services for Community Support Program (CSP) and Comprehensive Community Services (CCS) consumers, including assessment, service planning, coordination, and court-related proceedings under Chapters 51 and 55.

Responsibilities

• Complete comprehensive assessments for CSP/CCS consumers to identify strengths, needs, values, priorities, and recovery goals.
• Develop recovery-oriented, individualized service plans addressing consumer needs and goals identified during assessments.
• Monitor service plans for progress toward goals and consumer satisfaction.
• Provide service facilitation and coordination to community agencies for consumers.
• Facilitate regularly scheduled team meetings for consumers and their recovery teams.
• Arrange appropriate placements for clients as clinically necessary.
• Record clinical treatment and progress notes according to program standards.
• Prepare and present client cases for clinical supervision.
• Monitor inpatient treatment plans and develop discharge plans for assigned clients in psychiatric facilities.
• Facilitate client transportation when appropriate and conduct home visits as needed.
• Assist with client crisis or emergency situations.
• Conduct interviews with clients, families, law enforcement, medical personnel, and others to assess problems under Chapters 51 and 55.
• Assess situations and determine appropriateness of referral to Corporation Counsel for legal action.
• Screen information and prepare dispositional recommendations for Corporation Counsel.
• Initiate court proceedings, arrange witnesses, testify at hearings, complete court reports, and make treatment/facility recommendations to the court.
• Provide consultative services to family members before and after hearings regarding process, options, and results.
• Provide case management and follow-along services to post-commitment clients.
• Formulate recommendations for dismissal or recommitment to Corporation Counsel in conjunction with treatment staff.

Requirements

• One year of experience within the last ten years working with persons diagnosed with a mental disorder.
• Certification or eligibility for certification as a Social Worker in Wisconsin preferred.
