About the Role
This position focuses on building and maintaining relationships with local Community Recovery Centers (CRCs), inpatient psychiatric facilities, community providers, neighborhood associations, and both short- and long-term residential providers to drive program growth and support participants.
Responsibilities
- Assess the needs of individuals, families, or communities to identify necessary resources and services.
- Connect clients with appropriate community resources and refer them to services to aid in their recovery.
- Advocate for and assist clients in navigating social service systems.
- Organize and promote community outreach programs and workshops to educate the public on available resources.
- Collaborate with social workers, healthcare professionals, and community leaders to address broader community issues.
- Document and maintain records of clients' cases, including services provided, assessments, and progress.
- Provide crisis intervention and counseling in situations requiring immediate social service attention.
- Develop community programs addressing issues such as homelessness, substance abuse, and domestic violence.
- Work with other social workers, counselors, and health professionals to provide optimal care for clients.
- Monitor and evaluate clients' progress, modifying treatment plans as needed.
- Support administrative staff, volunteers, and students.
- Attend community resource and networking events.
- Participate in community and home visits to established program members.
- Perform presentations and distribute information to community providers.
- Establish goals and objectives for the intake unit.
- Provide support for transitioning clients into services.
- Follow up with clients to ensure their needs are met and evaluate the effectiveness of service plans.
- Coordinate services with referral sources and other community resources to ensure timely treatment.
- Attend and participate in meetings, coordinating information and care requirements with other providers.
- Assess daily admission and discharge program status.
- Monitor referral volume to ensure timely processing.
- Improve professional skills through continuing education, peer supervision, and individual supervision.
- Refer clients and/or family members to community resources as necessary.
Requirements
- Bachelor’s degree in social work (BSW), psychology, or a related field; Master’s degree in social work (MSW) or psychology preferred.
- Minimum of 1 year of experience in community work with nonprofit/profit groups, organizations, or volunteerism.
- Counseling experience with adults and/or families diagnosed with mental illness and/or substance use.
- Valid FBI clearance, criminal history check, and child abuse history clearance required.
- Completion of all CBH mandatory in-services within three months of hire and annually thereafter.
- Valid Pennsylvania Driver’s License.
- Knowledge of federal, state, and local laws and regulations (e.g., HIPAA, state regulations, county mental health regulations).
