Social Worker Case Manager

About the Role

The Case Manager Social Worker (LMSW/LCSW) collaborates with the clinical team to coordinate patient discharge plans, aiming to enhance patient outcomes and ensure efficient healthcare utilization.

Responsibilities

• Conduct assessments to identify patient care needs and develop discharge plans, especially for high-risk patients.
• Work closely with healthcare providers to implement discharge strategies.
• Stay updated on healthcare trends, regulations, and payer requirements related to patient care and discharge planning.

Requirements

• Master’s Degree in Social Work (MSW) required; LMSW certification necessary, LCSW preferred.
• Minimum of 2 years hospital social work experience, including post-graduate internship or related roles.
• Valid MD Social Work License appropriate to education level.
• Effective verbal and written communication skills, with the ability to follow instructions.

Schedule

Full-time position involving three 12-hour shifts per week, from 7:00am to 7:30pm, including weekends.
