Job Summary and Qualifications

  • Responsible for promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified patient populations.
  • A member of a multidisciplinary health team, acts as a liaison between the patient and community resources/post-acute care providers.
  • Creates and facilitates the timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care.
  • Facilitates patient movement across the continuum of care.
  • Collaborates with the health care team and develops treatment goals and plan intervention that assists the patient and family to resolve psychosocial concerns.
  • Provides crisis and supportive counseling to enhance the patient’s and family’s problem solving and coping skills, and counsel patient/families regarding advance medical directives.
  • Documents patients’ social work assessments, treatment goals, interventions, outcomes, and discharge plans.
  • Refers patients who require extended care post discharge to long-term care facilities, and once an appropriate bed is located, arranges for the patient’s transfer.
  • Identifies, tracks, and records Alternative Care Days (ACDs), intervenes to prevent or decrease ACDs as appropriate.
  • Performs admission and concurrent review for all payers and maintains legible documentation as directed by the Utilization Management Plan.

Qualifications

  • Graduate of an accredited school of social work.
  • Bachelor degree in Social Work BSW.
  • 2 or more years of medical social work experience in an acute care setting preferred.
  • Previous case management or utilization review experience preferred.
