Responsibilities
- Takes referrals from primary care providers and works with the primary care team to accomplish tasks related to social support navigation for social determinants of health (SDOH).
- Compiles and maintains a resource list for SDOH resources including eligibility criteria, referral process, and contact information.
- Collaborates with primary care nurse and providers to provide in-person or remote social needs screening/assessment with primary care patients referred by nurse or provider.
- Coordinates or makes aware of social services resources, such as housing, clothing, food, mental health services, etc.
- Collaborates with other social workers to identify patient and community resources and conduct case management activities.
- Works with hospitals for discharge planning, follow-up, and education.
- Aids in obtaining patient records from hospitals and securing needed medical equipment through community partners.
- Follows up on care plans and identifies patients lost to follow-up or overdue for care and assists them in returning to care.
- Assists with specialty referral navigation, scheduling, and tracking non-BCS specialist and imaging referrals.
- Aids in obtaining patient records from specialists and imaging centers and compiles and maintains a resource list for specialty referrals including eligibility criteria, referral process, cost, and contact information.
- Aids in assisting patients with locating and accessing low-cost prescription options such as patient assistance programs, discount retailers, etc., and assists with patient assistance program applications and serves as a patient-provider liaison with the drug companies.
- Aids in helping with other regional primary care-based initiatives with a social work component.
- Documents in patient’s record, updates consults, and tags provider and/or clinical staff as necessary.
- Provides patient education or finds appropriate education resources.
Qualifications
- Bachelor's degree in social work is required.
