Social Worker - Care Management II - Adult IOP

About the role

Kettering Health Behavioral Medical Center in Moraine is seeking a Social Worker - Care Management II for a full‑time position working 10‑hour first‑shift hours. Kettering Health is a not‑for‑profit system of 14 medical centers and more than 120 outpatient facilities serving southwest Ohio. Responsibilities

• Conduct comprehensive psychosocial assessments.
• Provide counseling and support to patients and families.
• Coordinate care plans addressing social, emotional, and resource needs.
• Collaborate with interdisciplinary teams to facilitate safe transitions of care.
• Advocate for patient needs and connect patients to community resources.
• Perform other duties as assigned.
• Ensure all responsibilities align with Kettering Health's mission, vision, values, and B Safe program.

Requirements

• Master's degree in Social Work (MSW).
• At least 1 year of clinical social work experience in a healthcare setting.
• Current Ohio Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW); must maintain active licensure per Ohio state law.
• Basic Life Support (BLS) certification, or ability to obtain within 90 days of hire.

Qualifications

• Experience in care management, discharge planning, or oncology social work (preferred).
• Certification in case management (e.g., Accredited Case Manager [ACM] or Certified Case Manager [CCM]) (preferred).
